MHSA member agency Project Hope is seeking applicants for a Health Starts at Home Housing Case Manager/Care Coordinator. Reporting to the Director of Housing Services, the Care Coordinator will be responsible for providing housing and case management assistance to families who are experiencing housing instability. The Care Coordinator will be responsible for assessing each family, linking them to needed services and providing follow-up case management services to ensure stability.

The Hearst Health Prize, in partnership with Jefferson College of Population Health, is given in recognition of an individual's or organization's outstanding achievement in managing or improving health in the U.S. The winner will be announced on March 20 at the 18th annual Population Health Colloquium.
